The paperwork for camp is provided below. These documents are available in Adobe® .pdf format. Camper Registration Form for 2009: This form must be filled out for ALL campers!

Payment Instructions: Look at this document for payment information if you live outside the United States.

Scholarship Application: This form is for campers who may need a leg up to attend Camp Marist.

Checklist & Summary: Use this sheet to doublecheck that all necessary forms are completed and to make arrangement for any final balance on your account.

Parent Input Form:  This form helps the staff to learn more about the child outside the camp.

Activity/Special Trip Form:  This form helps parents and children select activities and special trips that they can participate in while at Camp.

Arrival/Departure Information Form:  This form keeps us informed on how your child will be getting to arriving at and departing from camp and whether we will be providing transportation.

Authorized Persons Permission Form:  This form tells us who, if anyone, is allowed to contact, visit your child while at Camp and who, if anyone, is allowed to remove them from camp.

Air Travel Information Form:  This form need only be filled out if your child will be arriving by Air.

Required Uniform Letter:  Print this letter as a reference to assist in ordering required tee-shirts that your child must were when taken off property.

Medical Form:  This form MUST be thoroughly completed by parents and by a physician before your child can come to camp.
